Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the Federal District Court for your judicial district (or, if outside the United States, for any judicial district where Build Wealth Academy may be found).

A statement under penalty of perjury that you have a good faith belief the material was removed or disabled as a result of mistake or misidentification.
Identification of the material that has been removed or to which access has been disabled and the location where it appeared before removal.
Upon receipt of a valid counter-notification, Build Wealth Academy may restore the removed content within 10–14 business days, unless the original copyright claimant files a court action to prevent restoration.
Repeat Infringers
Build Wealth Academy reserves the right to suspend or terminate user accounts found to be repeat infringers or who repeatedly violate the intellectual property rights of others.
